Pain at Rest

Daily

Self-reported pain intensity when not moving. 0 = no pain. 10 = worst imaginable. Tracked every morning and after every workout.

The Numeric Pain Rating Scale (NPRS). The most widely used clinical pain measure. A 2-point drop is clinically significant. We're tracking it daily — at rest and under load — to show the real trajectory.

Heart rate variability — the time between heartbeats. Higher HRV = better nervous system recovery. Tracked via wearable overnight.

HRV measures the variation in time between each heartbeat, controlled by your autonomic nervous system. Higher HRV means your body is recovering well and handling stress. Lower HRV signals overtraining, poor sleep, or nervous system strain. Elite athletes track this obsessively.

PHQ-9

Patient Health Questionnaire — 9 questions screening for depression severity. Scored 0-27. Tracked weekly with Dr. Adam.

The PHQ-9 is the gold-standard depression screening tool used in clinics worldwide. 0-4 = minimal, 5-9 = mild, 10-14 = moderate, 15-19 = moderately severe, 20-27 = severe. A 5-point drop is clinically significant improvement.

GAD-7

Generalized Anxiety Disorder — 7 questions screening for anxiety severity. Scored 0-21. Tracked weekly with Dr. Adam.

The GAD-7 is the standard anxiety screening used in primary care. 0-4 = minimal, 5-9 = mild, 10-14 = moderate, 15-21 = severe. Tracks how much anxiety has interfered with daily life over the past two weeks.

PTSD Checklist for DSM-5 — 20 questions screening for post-traumatic stress. Scored 0-80. Tracked weekly with Dr. Adam.

The PCL-5 is the standard PTSD screening tool. It measures how much trauma symptoms have bothered you in the past month across four clusters: re-experiencing, avoidance, negative alterations in mood/cognition, and hyperarousal. A 10-point drop indicates clinically meaningful change.

Total sleep duration and quality stages (deep, REM, light). Tracked via wearable. Goal: 7+ hours of quality sleep.

Sleep is when the body repairs at the cellular level. We track total duration plus time in deep sleep and REM. For recovery from invisible disabilities, sleep quality is as important as any active protocol. Without sleep, nothing else works.

Blood oxygen saturation — how well oxygen is circulating. Normal is 95-100%. Below 90% needs attention. Tracked via finger sensor overnight.

Peripheral oxygen saturation measures the percentage of hemoglobin carrying oxygen. Critical for athletes with sleep apnea. Drops during sleep can indicate breathing interruptions that steal recovery. We track this alongside sleep data.

Pain at rest and under load. Baseline established Week 1. Weekly logged after each Coach Joe session and Catalyst appointment. Goal: 50% reduction by Day 90, with measurable drops tracked every single week.

Measured by Manny (physio) and Trent (chiro). FMS screen, goniometer readings, filmed assessment. Goal: Objective improvement cervical, thoracic, and lumbar — tracked and compared to baseline at every session.

Mental Health

PHQ-9 (depression), GAD-7 (anxiety), PCL-5 (PTSD). Three gold-standard clinical screenings. Weekly with Dr. Adam. Goal: Clinically significant improvement across all three scales — measured, scored, and published.

Testosterone, cortisol, vitamin D, inflammatory markers. Quarterly draws at baseline, Day 45, and Day 90. Hard biochemistry. No subjective interpretation. The blood tells the truth.
